How Can I Find Out Which Doctors Accept HealthPerxPlus?

For Your Dental and Vision needs you will receive a packet within a few weeks that contains a detailed listing of all professionals and facilities in the area in which you live. Our Patient Care Coordinators will walk you through the process giving you information on the providers that according to our customers provide the highest level of satisfaction. For your Dental, Vision and Prescriptions Drugs benefit simply show your card at the point of service. Calling ahead the facility will not usually give you information regarding pricing. Pharmacy Benefit is available at over 60,000 pharmacies nationwide.

How Can I Qualify For HealthPerxPlus?

Household means any person who is dependent upon you financially. Yes, this can be your children up to age 27 who reside at home or are enrolled in a college or University. If you have parents or other loved ones who are financially dependent on you they will also qualify for coverage under your family plan.
